Salesperson License Application "Part A"
This form is Part 1 of 2 of the initial application for a real estate salesperson license. This form is designed for use by resident, non-resident and reciprocal salesperson license applicants.

Salesperson and Associate Broker "Part B" (Sponsorship Affidavit)
This form is Part 2 of 2 of the initial application for a salesperson or associate broker license.

Branch Office License Application NEW FORM
An office established for conducting any real estate license activity at an address other than the registered address of the broker, wherein the name and telephone number of the broker or agency is advertised in any way, shall be considered a branch office and shall be licensed as such.
